Downloadable Forms Parish Census Form To become a registered parishioner of St. John please print out this form, fill out all information paying special attention to the key at the bottom of the page. Also please note the area under the Head of House/Spouse section that asks for information about your marriage. This section often gets overlooked. E-mail to smoreau@eatel.net Stewarship of Ministry Form This form is used to sign up for the various ministries in the parish. This form should be returned to the parish office. This form enables us to draw from a computer database as needed. E-mail to smoreau@eatel.net Offertory Direct Debit Form This form is used to have your weekly or monthly offering debited from your checking or savings account. Please foolw directions on the form and return to Karen Hagendorfer in the parish office. Khagendorfer@eatel.net Baptism Sponsor/Godparent Form These forms must be filled out by the God parents to be and signed by their parish priest. The forms must be in before Baptism can take place. Smoreau@eatel.net Baptism Registration Form This sheet must be completed in order for a child to be baptized in the parish. Please mail it or drop it off at the parish office. smoreau@eatel.net Dates for Baptism and Baptism Seminars Baptism Guidelines This document details what is necessary for Baptism in this parish as well as giving the dates of Baptism seminars and scheduled Baptism dates. Christian Formation Service Hours Form This form is for students in grades 9-12 to record service hours. Forms are to be turned in to Mr. Shows upon completion. Confirmation Sponsor Eligibility Form This form is to be downloaded and filled out by the Conformation candidates sponsor. It is to be signed by their pastor and returned to the Christian Formation office. Saint Name Form This form is to be used by the Confirmation candidate. The candidate should research the saints and choose one that best suits them. Youth Registration Forms This form is for students in grades 5-8 and 9-12. All forms should be turned in to Ashley Bourgeois in the parish office EAPS Background Application This is the form that must be filled out in order to work with children in the parish. This includes all areas of Christian formation and Youth Ministry. Code of Ethics and Behavior -This must be read in its entirety. Safe and Sacred - Web site for Child safety. You must create a username and log-in on your first visit. After completeing all the requirements you will take a quiz online and when passed you will need to print it out and return it along with the EAPS Application to the parish office. |
